Summary
We are committed to providing adult care and integration services that put the person at the centre of our work. We are passionate about following the ‘Home First’ pathway and work closely with our hospital colleagues to achieve this.
We cover a range of areas such as intermediate care and Athorpe (non-acute bed bases) as well as community reviews and supporting with longer term reviews. We work collaboratively with health partners in the Transfer of Care Hub.
Successful candidates will join supportive and established team which is comprised of nursing and social work professionals based at Woodside. As part of this role, you will be expected to support others in the team, including newly qualified Social Workers and Assessment and Review Coordinators to develop their skills and experience.
