Summary
Main Duties of the Job As part of our team, you will contribute to a range of medicines optimisation and patient safety initiatives. Your key responsibilities will include: Supporting medication reviews and medicines reconciliation Helping patients understand and use their medicines effectively and safely Promoting safe and effective repeat prescribing processes Assisting with audits, service improvement projects and prescribing initiatives Liaising with community pharmacies and secondary care providers to support continuity of care Playing an active role in reducing medicines waste and improving prescribing outcomes
