Summary


The post holder will be expected to be a responsible individual who will work in conjunction with the nursing team to assess patients at the point of admission and assist in the collection of the clinical and social information required to plan for individualised care and treatment. You will work independently at times to maintain flow and efficiency through the department, being involved with transfers and discharge planning. We require you to act as a role model for more junior health care assistants by supporting them to deliver a high standard of care to our patients.



· Providing personal care to patients

· Handing out meals & help with feeding patients

· Completing observations and escalating to the nurses if required

· Filling out the care plans

· Keeping general areas tidy and clean e.g. kitchen

· Venepuncture and Cannulation

· Monitoring confused or mental health patients

· Mentoring Band 2 HCA’s

· Liaising with other multidisciplinary teams e.g pharmacy, physiotherapists, doctors

· Escorting patients to wards, CT and other appointments

· Booking transport

· Communicating with relatives & carers

· Assisting with the Consultant Post Take Ward Round

• Staff who provide basic care and support to patients in various settings
• Responsible for tasks such as hygiene, mobility, feeding, observations, and medication
• Involved in keeping the environment and equipment clean and safe
• Part of the wider healthcare team that includes nurses, doctors, and other professionals
• Taking patients observations such as blood pressure, temperature, oxygen saturations and pulse.
• Helping patients with meal times and feeding.
