Summary

The Frailty Nurse specializes in assessing, managing, and supporting older adults who are living with frailty. This role focuses on preventing deterioration, promoting independence, and coordinating holistic care to improve patient outcomes in clinical setting. Key Responsibilities: Conduct comprehensive assessments to identify frailty and related health risks. Develop and implement individualized care plans tailored to the needs of frail patients. Monitor patients physical and functional status, adjusting care plans accordingly. Collaborate with multidisciplinary teams including doctors, therapists, social workers, and caregivers. Provide education and support to patients and families on managing frailty and maintaining wellbeing. Facilitate access to community resources and services to support patients needs. Promote best practices in frailty management and participate in continuous service improvement. Maintain accurate, confidential patient records in line with healthcare standards. Support the prevention of hospital admissions and readmissions through proactive care. Qualifications: Registered Nurse (RN) with experience in geriatrics, community, or primary care nursing. Specialized training or certification in frailty assessment and management preferred. Strong clinical and assessment skills with a holistic approach to patient care. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Compassionate and patient centered approach.
