Summary
The post holder will: Behave consistently with the values and beliefs of the organisation and promote these on a day-to-day basis. Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions to patients either in their own homes or other community settings. This will be achieved through a combination of hands on expert clinical skills and knowledge as a Physiotherapist to treat, educate and support patients and by working with patients and their families to assess, plan and implement appropriate care plans and treatment. Provide case management for a designated caseload of patients) who have immediate and / or complex care needs. Undertake the comprehensive assessment and diagnosis of patients including those with a highly complex presentation, using investigative and analytical skills. Formulate individualised management and treatment plans using clinical reasoning
