Summary


This postprovidesan exciting opportunity for ahighly motivated and enthusiastic personinterested in workingwithin Cancer Care.


The post holder will be expected tosupport the Breast Care Nursesto ensure the smooth,safeand efficient running of the team. As part of themulti-disciplinary team the post holder will contribute to the efficient seamless service provision with a high standard of care by providing elements of direct and indirect patient care and clerical work.

To work closely with the nursing team and wider MDT, triaging emails and telephone queries.

To organise and setup Holistic Needs Assessments on newly diagnosed and end of treatment patients.

To manage and coordinate prosthesis clinic and ordering of prosthesis.

To be responsible for preparing the surgical and oncology clinics and to ensure that the scans are chased in a timely manner.

To be able to assist in managing and co-ordinate patients on a Personalised Stratified Pathway (PSFU).

To have responsibility for adding and monitoring patients who are managed on a remote monitoring system (Info flex).

Communicate effectively with relevant MDT’s and clinical teams to ensure that all new and migrated stratified follow up patients are referred for discussion after assessment by health professionals.
