Summary
Key Responsibilities: Provide high-quality, evidence-based care for a broad range of medical conditions in a general practice setting. Provide support for other GP colleagues by taking a share of admin such as prescriptions, and own admin work (Docman and pathlinks) (time allocated for this) One Home Visit per DAY with allocated time. Work closely with multidisciplinary teams across the PCN, including pharmacists, , social prescribers, and physiotherapists. Contribute to routine general practice duties including face-to-face consultations, telephone triage, home visits, repeat prescription reviews, and QOF achievement. Maintain accurate patient records using EMIS Attend regular clinical meetings and participate in audit and CPD activities if appropriate. Essential Requirements: GMC registration with a license to practise. Member of the RCGP and on the GP Performers List. Qualified as a GP within the last 2 years. Commitment to collaborative and patient-centred care. Enthusiasm for working in a multidisciplinary environment.
