Summary
Role Purpose: The Healthy Lifestyle Advisor Lead role is designed to work closely with the wider management team and provide additional support to the team and operations within the services. The role will require leadership qualities and will hold accountability towards successfully driving service outcomes. The role will carry an element of delivery, where agreed, to cover for team absence and promote service growth. You will join a team comprising of a GP, Head of Service, therapists, nutritionist, counselling specialists and administrators. You will be involved in providing management support and contributing to MDT meetings, supervising and supporting the delivery element of the service, leading clinics and supporting group education and intervention sessions. Development of resources and staff training is also included. The interventions can occur via telephone, digital platforms or face-to-face community venues across the footprint. You will be supported by the Obesity lead, service manager and the wider MDT. Your key responsibilities will be: Managerial To assist the Operation Team Leads/Specialist Leads in the day to day running of the project To be a point of contact for delivery staff queries Assembling and coordinating staff to facilitate project delivery: overseeing staff calendars; liaise with the admin team, coordinate staff to provide session cover when necessary. Lead on developing certain areas of the project according to the project plan Accountability towards tracking and achieving service KPIs Undertaking service observations and providing valuable performance-driven feedback Support the line management of staff by performing staff 1:1s and PDPs Oversee and monitor stakeholder activity and targets across all areas - liaise with stakeholder staff regularly, track performance according to service KPIs, work with the management team to set targets to improve performance Undertake data preparation and analysis as requested by management and attend key monthly meeting for internal and external performance board Work with system partners and stakeholders to design, deliver and evaluate the service engagement Track and manage patient flow and outcomes through the system, as requested by the Operations Team Lead to put actions in place for those who are not progressing as they should To coordinate the booking, risk assessing and use of various delivery venues Support with financial duties including the processing of staff expenses and invoices relating to the project Lead on the updating of data where data completeness is an issue Audit of data quality to ensure that patient data is up to date and identify issues with data completeness and validity: oversee and coordinate monthly Contribute to clinical governance procedures Constantly monitor and report on progress of the project, to present reports defining progress, problems and solutions Contribute to training and support of new staff and volunteers Delivery Deliver safe personalised, age appropriate, one-to-one group exercise interventions, behaviour change and physical activity advice to children, young people and families Prepare and set up venues in a safe manner Have a thorough understanding of the curriculum/teaching resources and be confident in the delivery of such. Session planning including preparing the correct handouts, materials and equipment for the session Discussing weight loss / gain with clients Monitor weekly weight loss / gain and attendance against set targets. Client contact calls to ensure attendance, chase non-attendance Service / client evaluation Oversee curriculum development To support a team of Healthy Lifestyle Advisors to deliver a quality driven service spanning a range of interventions within the IWS. To engage and recruit potential clients at promotional and community events. Cover sessions where necessary. Training The postholder must attend any training that is identified as mandatory to their role.
