Summary
Education & professional Qualifications: Nurse/Social Worker/AHP with Professional Registration from appropriate governing body e.g. NMC, HCPC Relevant degree Evidence of C.P.D in relevant areas of Practice Post registration experience relevant to clinical area Experience in care of the Elderly, Frailty and acute medicine Experience working in an acute health setting and complex discharge planning. Experience: Excellent clinical reasoning /clinical decision making Ability to manage a diverse workload and to meet set deadlines Ability to work on own initiative and without supervision.
