Summary


To work as a member of a multi–disciplinary team assessing social needs on an individual and group basis for peoplewith learning difficulties and/or autism who are currently accessing services from Amber Lodge Low secure unit.


• Communicates effectively in an MDT setting and promotes strengths based human rights-based practice and the social model of Learning disabilities/mental health. Is adapt at communicating internally and across organisations in relation to safeguarding both in terms of child protection, vulnerable adults and complex cases.
• Regularly communicates with a wide range of other statutory and voluntary sector professionals. Produces reports for legal processes as part of duties in respect or the Mental Health Act, Care Act and Mental Capacity Act.
• Is able to assess and manages risk, support in relation to personal budgets, home care and where required undertake referrals and admissions to care, using a strengths-based approach to assisting those who use care and support services to meet their goals.
• Support and provide information and assessment to carers.
• Understands the needs of those accessing Amber Lodge
• Undertakes strength based assessments of individuals social care needs within a legislative and governance framework.
