Summary

Hales Home Care is seeking experienced Care Workers / Care Assistants / Carers to join our Discharge Support Service across Gateshead.

Areas covered:

Zone 1 – Felling, Windy Nook, Leam Lane, Deckham, Old Fold, Bensham, Teams.
Zone 2 – Wrekenton, Birtley, Harlow Green. Zone 3- Dunston, Whickham, Sunniside, Blaydon, Crawcrook, Ryton, Prudhoe.

Shifts or hours available:

You will be required to work on a rota basis between 7am to 11pm, 7 days per week.
7am to 2:30pm, 2.30pm to 11pm.
4 on 3 off shift pattern.

To be successful in this Support Worker role, you will need a full driving licence and access to your own vehicle, as well as previous experience in providing person-centred care. This involves dynamically assessing the needs of individuals and supporting their transition from a hospital stay to living as independently as possible in their own homes.

You will be required to liaise with other health professionals, and signpost service users and family members to the appropriate additional support, equipment and services to aid their ongoing recovery and long term care needs.

The Ideal Hales Care Assistant

Good communication skills – verbal and written to complete accurate records
Previous experience in care, retail, hospitality or other service industry is welcome but not essential.
Full UK driving licence and access to a vehicle required.
