Summary
Job Title: External Digital and Transformation Lead Reports to: Clinical Director Hours: 15 hours per week Location: Central Hub, Rainham Salary: Band 8a (depending on experience) Purpose of the Role The External Digital and Transformation Lead plays a vital role in ensuring the smooth, efficient delivery of services from our central hub. This includes operational oversight, staff management, digital system leadership, and service improvement. The postholder will work in close partnership with the Internal Digital and Transformation Lead to co-deliver strategic goals and drive innovation across the Primary Care Network (PCN). The role involves collaboration with a wide range of stakeholders, including GP practices, community services, ICBs, and local partners, with a shared focus on improving patient experience, access, and population health outcomes.About Us We are a proactive and patient-focused Primary Care Network (PCN) located in central Rainham, made up of 8 GP practices and serving a population of over 38,000. We deliver a wide range of enhanced services, including: Clinical pharmacy Diabetes and respiratory management Same-day paramedic home visiting Care coordination Social prescribing Mental health support Minor illness clinics Podiatry Physiotherapy, including advanced physio-led steroid injections We are proud to be an accredited clinical learning environment, hosting student nurses and paramedics on placement. As an active member of the Integrated Neighbourhood Team, we work closely with system partners to improve health outcomes and reduce inequalities. Main Duties and ResponsibilitiesService Delivery & Operations Lead the day-to-day delivery of services from the central hub Manage rotas, staff deployment, and logistics to ensure service continuity Identify and resolve operational issues that affect access or quality Maintain compliance with clinical, contractual, and quality standards Line Management Line manage a range of clinical and non-clinical staff Provide supervision, support, and performance management Support staff development and contribute to a positive team culture Strategic & Digital Transformation (Co-delivered with Internal DTL) Support the design and implementation of service improvement initiatives Lead the operational use of digital systems (EMIS, Ardens, Apex, KMCR) Contribute to the rollout of new technologies and digital inclusion efforts Participate in the planning and delivery of PCN transformation projects Stakeholder Engagement (Co-delivered with Internal DTL) Build and maintain strong working relationships with PCNs, ICBs, and community partners Represent the organisation at local forums, stakeholder events, and neighbourhood team meetings Oversee mechanisms for capturing and responding to patient feedback Performance & Quality Improvement Contribute to delivery of IIF, GPAD, and Access targets Use data tools to monitor service impact and inform decision-making Support continuous improvement and innovation in care delivery Cross-Cover & Collaboration Provide occasional cover for the Internal Lead (e.g., HR, finance, shared platforms) Foster a collaborative, joined-up approach to leadership and delivery across both DTL roles Key Relationships Clinical and service delivery staff Practice managers and PCN member practices Peer Digital and Transformation Lead (Internal) Integrated Care Board (ICB) partners Community and voluntary sector organisations Digital and system suppliers
