Summary
We are looking for a motivated, passionate and experienced Paediatric Palliative Care Clinical Specialist Nurse to work within the Community Children`s Nursing Service (CCNS) which is part of the Family & Therapies Division with Aneurin Bevan University Health Board. Adopting a family centred care approach, this role involves supporting families from diagnosis through end-of-life care and bereavement, collaborating with multidisciplinary teams and empowering staff to deliver excellent nursing care delivery in line with evidence based best practice. The post holder will work with families and members of the multi-disciplinary team, liaising with: -inpatient, community and hospice services to co-ordinate and support the delivery of high-quality -holistic care to babies, children and young people who have life limiting or life-threatening conditions, by providing clinical expertise, supporting complex decision making, advanced care planning andsymptom management.
