Summary
To act as a Highly Specialist Clinician, responsible for the assessment and treatment of patients with complex palliative care needs in the community. Providing advanced level assessment and intervention for patients with life limiting illnesses referred into the service. This may include cancer, degenerative neurological conditions, heart failure, and dementia. To provide expert advice and act as a source of expertise in the management of multiple complex conditions. To provide guidance to other health care professionals and members of the MDT. To act as a lead clinician within specialist area, supporting the operational delivery of the service within CCICP. You will be responsible for service development initiatives and drive service developments, while ensuring consistent service delivery within CCICP. To maintain and develop links with the wider MDT to support and improve therapy pathways between hospital and community settings within the Trust.
