We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.Accommodation to us doesn’t just mean a roof over your head and shelter from the elements. At Changing Lives, we offer a home rather than just a shelter.About the ProjectOpen House Plus, located in Doncaster is a 26 room homeless hostel. Our project provides supported accommodation for individuals with a number of complex needs. As a Support Worker at Open House Plus, you will have the opportunity to join our passionate team who are dedicated to empowering those who we supportWe are committed to delivering high-quality support, and we welcome your expertise and dedication in order to help us to continue making a positive impact in the lives of those we support. You will be given training on all aspects of the role as well as taking part in case management reviews with the whole team.Our project operates on a shift schedule; therefore, flexibility is essential as shift times may vary to ensure uninterrupted support. The successful candidate would typically be working one week of day shift followed by one week of afternoon shift and incorporates working alternate weekends. Day shift 07:30 - 15:30. Afternoon shift 14:30 - 22:30Role SummaryThe Support Worker plays a crucial role within Changing Lives, providing information, advice and guidance, which is undertaken via referral and direct individual support. The Support Workers aim is to empower the people we support to live independently and work towards overcoming the interconnected problems that lead them into accessing our services.Key activitiesYour key activities will include:Working both individually, and as part a team, to build productive relationships with individuals accessing the Changing Lives service.Support, listen, guide and signpost individuals to allow them to receive the required support to manage their physical environment.Ability to build relationships on 121 basis, as well as within a group environment, to provide key support and enhance personal wellbeingCarry out regular building health and safety inspections, identifying risk factors, responding accordingly, and carrying out basic repairs where required.To ensure that the standards of the building are maintained by following designated tasks, whilst maintaining a high level of cleanliness, meeting all legal and statutory requirements to operate safely, effectively and efficiently.Empower individuals to improve their life skills through the delivery of workshops/activity sessions and/or signpost to external agencies who can support in this area.Ensure that full written reports, documentation and internal systems are updated accordingly, in line with Changing Lives' policies and procedures, whilst always maintaining a high level of confidentiality and adhering to GDPRSupporting residents to gain independent living skills such as cooking activities, shopping, use of the washing machine.Supporting residents to appointments within the community such as attending probation, drug and alcohol services, doctors' appointments etc.Building robust move on plans and supporting residents to independent living Leading on activities to engage residents in meaningful use of time.Signposting and making referrals to other agencies such as domestic abuse services, drug and alcohol services, mental health etc.Raising safeguarding referrals, MARACS, MAPPA and attending multi-agency meetingsCarrying out allocated daily cleaning tasks
