This is an exciting time to join Calderdale Council’s Adult Social Care Teams. Calderdale Council is the most improved adult social services Directorate in Yorkshire Humber. An opportunity has arisen to join the Mental Health Social Work Team. The Mental Health Social Work Team is a well-established team of experienced Social Workers, delivering statutory social work interventions for adults of all ages living with a mental health condition. The team support people with a wide variety of needs whether this be during a brief period in someone’s life or on a longer-term basis. The team are committed to working collaboratively with partner agencies and community resources, to ensure people receive the right support from the right place at the right time.

We are seeking an individual who is passionate about adult social care and providing a high-quality service and is keen to develop their professional experience and knowledge. The Service Coordinator role will be expected to undertake care act assessments, develop and review support plans amongst other duties.

The Service Coordinator will be a key part in ensuring our continued success in fulfilling the Council’s statutory responsibilities under the Care Act 2014. The service is committed to helping people to achieve the outcomes that matter to them in their life and the successful applicant will work closely with colleagues and partner agencies to protect an adult’s right to live in safety, free from abuse and neglect.
