Clinical Assess, plan, implement and evaluate person centred nursing care for a named group of patients in compliance with hospice policy and protocols. Undertake holistic assessment of patients, families and carers, including their emotional, physical, spiritual, practical, welfare rights and as appropriate pre and post bereavement needs. Ensuring that patients, their families and carers are involved in their plans of care. Actively support and plan the discharge planning and process. To develop, update, monitor and evaluate patient care plans ensuring they are person centred and support individual needs. To demonstrate the skills and knowledge that enable the delivery of basic and complex specialist nursing care interventions that are responsive to individual need. Have knowledge of common symptoms, anticipate change and evaluative interventions. To communicate professionally, sensitively and effectively with patients, carers, families, the multi professional team and all staff within the hospice. To deliver concise and accurate patient presentation in daily hand overs and to attend and participate the weekly MDT meeting in the absence of senior nursing team if requested. In the role of ward coordinator be responsible taking a nursing lead on admission and discharge planning as required. Take day to day responsibility for planning and organisation of the shift and prioritise effectively according to patient need, safety and staffing. To communicate effectively with own team and maintain effective communication between the MDT and external agencies. To communicate highly sensitive information, ensuring patients, their families and carers are suitably supported. To respond to helpline calls effectively and in a timely manner, including escalation to senior team members or appropriate signposting or advice is given and documented. To promote and support Advance Care Planning. Ensure accurate and timely records of care (electronic and paper) are maintained, upholding compliance with information governance, being responsible for having a SystmOne card on all shifts. Follow all agreed policies for manual handling, infection control, health and safety and security. Failure to adhere to these guidelines causing harm to a patient, family member or hospice employee could lead to disciplinary action. Action the agreed policy in the event of a fire/fire drill. Ensure the dignity, privacy and confidentiality of all patients and families according to Caldecott principles, hospice policy and guidelines. Participate in audit and evaluation of clinical practice and lead on delegated areas of clinical development. Actively promote and engage in reflection and clinical supervision Actively promote a culture that supports effective team work. Support all students including nurse associate trainees.
