Summary
Key Responsibilities: 1. Patient Observations: - Conduct regular observations of patients, including monitoring vital signs, temperature, pulse, and respiration rates. - Report any changes in patients' health status to the healthcare team and record on patients notes. 2. Blood Sample Collection: - Perform venipuncture and capillary blood sampling as required. - Ensure samples are collected, labelled correctly, and transported to the laboratory in compliance with protocols. 3. Patient Care: - Make assessments of a patient's home surroundings, to offer social and mobility support. - Discuss Future Care plans, support comprehensive geriatric plans, and personalised care plans. 4. Administrative Tasks: - Maintain accurate records of patient observations and care delivered. - Ensure compliance with health and safety guidelines and organisational policies. 5. Collaboration with Team Members: - Work closely with the Non-Clinical Home Visiting Service Lead and other healthcare professionals within the service and Primary Care Network to coordinate patient care. - Participate in team meetings and training as required. You will be required to work from both our Hayling Island site and Emsworth site.
