Summary

Responsibilities for patient placement. -Facilitate placement of emergency and urgent admission requests, whilst recognising the importance of elective work. Ensuring there is sufficient capacity to assist in the achievement of both Trust and National performance targets. -The role will also require organisation of all internal transfers and repatriation to and from other Trusts. -Analyse the beds situation and, using skills, knowledge and experience, ensure that beds are allocated appropriately and effectively for emergency and elective admissions to maximize patient care and the patient experience -Ensure appropriate and effective allocation of beds to ensure that all patients are admitted to the correct bed first time -Maintain a 'bed state' that accurately reflects the position of St James Hospital during the span of a shift -To effectively communicate capacity and demand issues to the key staff as and when required . -Take personal responsibility for contributing to the patient flow teams working effectively, collaboratively and in a co-ordinated way. -To ensure that during their shift, the placement of patients into beds is both safe and appropriate and supports patient flow in both emergency and elective streams -Post holder will report to Clinical Matrons Site Operations both in and out of hours, providing senior support to ensure that best possible care is delivered in a safe manner. -Post Holder will work closely with an array of multi- disciplinary and speciality teams. To succeed in this role, you will need to possess excellent interpersonal and communication skills. You will also need to be able to deal with challenging situations in a fast-paced environment making independent decisions to avoid delays in the patient's journey, whilst acting in a calm and professional manner. -Minimise patient moves ensuring all infection control guidelines are followed Leadership and management- Investigate and respond to accidents, complaints, untoward incidents and other significant events - Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes. - Actively promote and use the Leeds Improvement method to improve the quality and safety of the service -- Promote evidence based practice, wherever possible - Engages users and carers views on service delivery and development - Implement policy and practice guidelines specific to the specialty, proposing changes where appropriate - Contribute to policy and protocol development for speciality area Communication -Maintains accurate clinical and other records using the Leeds Teaching Hospitals Trusts documentation. - Support patients and their families communicating sensitive condition related information and ensuring they receive required information. Education and Training - Provide training and education of learners and other junior staff - Actively participate in the development of own personal development plan and performance review - -Keep up to date with developments, advances within the specialty Health and safety /Risk management -All staff are responsible for working with their colleagues to maintain and improve the quality of services provided to our patients and other service users. This includes complying at all times with the Leeds Teaching Hospitals NHS Trust Policies, including Health and Safety policies, in particular by following agreed safe working procedures, and reporting incidents using the Trust Incident Reporting system. Infection control -The jobholder must comply at all times with the Leeds Teaching Hospitals NHS Trust Infection Control policies, in particular by practising Universal Infection Control Precautions. Hand hygiene must be performed before and after contact with patients and their environment. Training and personal development -Continuous professional -The jobholder must take responsibility, in agreement with his/her line manager, for his/her own personal development by ensuring that Continuous Professional Development remains a priority. The job holder will undertake all mandatory training required for the role. Respect for Patient confidentiality -The job holder should respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role.
