Summary

Prescribing and Medicines Optimisation Support the repeat prescribing process, including synchronisation of medications and managing prescription queries. Perform medicines reconciliation for patients post-discharge, ensuring accurate and safe updates to medication records. Review and process prescription requests, ensuring safety, legality, and cost-effectiveness in accordance with clinical guidance. Support medicines audits and switches, including those required for ICB formulary or safety updates. Patient-Facing Services Answer medicines-related queries from patients and carers, escalating where appropriate. Provide advice on the safe and effective use of medicines. Support patients with medication changes, including those linked to long-term condition reviews or deprescribing initiatives. Administrative and Quality Support Maintain accurate, up-to-date patient medication records. Support achievement of QoF, IIF and local medicines optimisation targets. Participate in the implementation of safety alerts and MHRA guidance. Support the practice or PCN with electronic prescribing, prescribing systems, and template development. Teamwork and Development Work collaboratively with GPs, pharmacists, nurses, and wider staff. Attend training and supervision sessions as required. Contribute to quality improvement projects. Maintain registration and keep up to date with CPD requirements.
