Summary

Management to facilitate and encourage a learning environment and be actively involved in meeting the educational needs of learners to be responsible for the care of equipment issued to a home and to report any defect or loss to participate in Child Protection procedures. to establish and maintain good relationships with the members of the multi-disciplinary team, for example, Acute services, Tertiary Centres, Social Services and other agencies to actively participate in the orientation and development of new staff to the community to participate in developing the CCN team Administration to maintain accurate records and statistical returns as required by Provide to participate in team and multi-agency meetings / conferences. to take appropriate action in regard to any accident or incident to a child, family member, staff member or visitor to be aware of Provide policies and act accordingly to maintain and update childs care plan in partnership with family and key professionals involved in that childs care. Personal Development to be aware of and act in accordance with the Nursing and Midwifery Council Code of Professional Conduct to participate in clinical and safeguarding supervision in accordance with Provide policy to participate in reflective self- appraisal and Personal Development Planning review to maintain up-to-date knowledge and skills and undertake education in accordance with individual, team and organisation needs, within a framework of a Professional Development Review to ensure personal and team safety by adhering to Provide policy on safety to be aware of and act in accordance with the organisations clinical standards and policies Communication Communicate condition-related information to the child and family/carers with empathy and reassurance. Establish and maintain effective communication with families and MDT on complex and potentially stressful topics in a range of situations. Develops, maintains professional relationships and communicates information to a wide range of health service and external staff as appropriate. Communicates issues relating to safeguarding children with the child, families and MDT. The post holder will be responsible for contemporaneous record keeping. There may be a requirement to work outside of normal working hours to facilitate end-of-life care at home with the expectation to support with on call duties which includes overnight and weekend cover.
