What We're Looking For:The ideal candidate will: Have an excellent skill mix in general practice nursing Be friendly, approachable, and flexible Have a good sense of humour and enjoy being part of a team Be confident in working across our three sites as required Be familiar with or willing to train on SystmOne clinical system If you're a dedicated and enthusiastic nurse who enjoys working in a dynamic, patient-focused environment, we would love to hear from you Key Responsibilities As a Practice Nurse, your main duties will include: Supporting health promotion and wellbeing initiatives Creating and reviewing care plans for patients with long-term conditions, Asthma, Diabetes, CVD etc Providing routine nursing care in line with best practice guidelines (e.g. wound care, health checks) Running chronic disease clinics (e.g. diabetes, asthma) and making referrals when needed Giving travel advice and vaccinations Taking bloods and requesting other tests as required Keeping clear and accurate patient records Ensuring correct coding and maintaining clinical registers Supporting with clinical emergencies when needed Giving vaccinations to adults and children Offering support around medications within your scope of practice Recognising and responding to mental health needs Offering health advice during patient contacts Helping and guiding junior nursing staff Taking part in team learning and local health initiatives Keeping up to date with national guidelines and best practice
