Crynodeb
1. To manage own caseload of Children with a Learning Disability, which will be identified bySenior Community Nurse for Children with a Learning Disability.
2. To plan care for Children with a learning disability and be a support to them and their families according to the care pathways.
3. To be a competent, autonomous community practitioner, with support from senior members of the Learning Disability Team.
4. To work as a member of Children and Young Peoples service to promote a seamless provision for children.
1. Toprovidecarein partnership with parents/carers/childrenusing interventionsappropriate tothe assessedneedas per theCNCLD Standard OperatingProcedures.
2.To evaluate the outcome of interventions using a process of regular reviewin collaboration with the child, family, CNCLDand otherprofessionals involved.
3.To advise on the promotion of health and the prevention of ill health.
4.ParticipateinSafeguardingChildrenprocesses asrequired, sharing information as necessary and completing reportswithin timescales.
