Summary
Please refer to the attached job description and person specification for full details of responsibilities Conduct specialist stroke nursing assessments and develop evidence-based care plans. Provide individualised patient-centred information on stroke diagnosis, investigations, and treatments. Offer secondary prevention advice and support interdisciplinary rehabilitation planning. Work collaboratively with acute stroke services, primary care, and community colleagues. Manage complex cases involving comorbidities and liaise with relevant professionals for optimal patient outcomes. Deliver emotional support and conduct mood assessments (e.g., PHQ-9, GAD-7). Identify rehabilitation goals using a multidisciplinary approach. Provide guidance on neuropathic pain, vision assessment, spasticity, and cognitive effects of stroke. Provide continence initial assessment & advice post stroke and make specialist referrals where necessary. Work across various settings, including patient homes, inpatient rehabilitation beds, care homes, and community locations.
