Summary


Enhanced Clinical Practitioner

An exciting opportunity has arisen within the Division of Intermediate Tier Services, we areseeking an Enhancedclinical Practitionerstojoin the Extensive Care service.

It is an exciting time to join our team as we are part of a fully integrated care organisation and work across five Neighbourhoods in Tameside and Glossop.


Extensive Care is an Umbrella term as the service hasa numberofsubservices within it.

The Extensivistserviceis wellestablishedover5years,andsees patient in the community who have multiplehospitalattendances, along with long termconditions and complex health needs. We arereally excitedto bealsosupporting and developing ouracute frailty service, with an ambition for an acute frailtypath wayinto the community.

The servicemain focusis to reduce unnecessary hospital attendances, treating patient at homewhere clinically safe to do so, whilst providing exceptional standards of care.

Main duties of the job

• Registered Nurse/ paramedic/Therapist withacurrent registration

• working atanautonomouslevel

• Independent non-medical prescriber

• Clinical examination course desirable


• Car and driving licence essential

• NMC Approved mentor preparation course or equivalent


The successful candidate will provide highly specialised care within the community and Care setting, to those patients with chronic and acute conditions using clinical knowledge that is evidence based


TheEnhancedPractitioner will function as part of an integrated Care organisation and willdemonstratecompetence in advanced assessment,managementand treatment.


Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the community.


Work collaboratively with other health care professionals in primary and secondary care to ensure a high quality easily accessible service.


Provide a community wide lead in the development of the role of theEnhancedpractitioner and the service provided


Tosupport newand follow-up referrals


To pro-actively support andmaintainpatients within the community and care home setting


Tofacilitatedischarge of patients back to their place of residence as appropriate


To undertake clinical audit
