Summary
Job Summary / Purpose: The Advanced Nurse Practitioner (ANP) is an integral member of our healthcare team, the ANP will provide a range of advanced clinical assessments, diagnostics, and interventions, ensuring high-quality care and patient-centred services working autonomously to support and work alongside the General Practitioners. As a key member of the primary care team, the ANP will have a strong background in general practice nursing and demonstrate exceptional clinical expertise, leadership, and communication skills. The role is diverse and will also include being involved in research, leadership and education as well as being actively involved in planning and implementation of projects and areas of practice development. Key Responsibilities: Conduct comprehensive patient assessments, including history-taking, physical examinations, and health screenings, to diagnose and manage acute and chronic medical conditions. Develop, implement, and evaluate individualised care plans in collaboration with patients and other healthcare professionals. Prescribe medications and manage pharmacological therapies according to established guidelines and regulations, while monitoring patient outcomes. Deliver high-quality care across the lifespan, addressing the needs of diverse patient populations, including children, adults, and the elderly. Provide health education and counselling to patients and their families, promoting self-management and preventive care strategies. Collaborate with GPs, nurses, administrative staff, and external providers to ensure seamless and coordinated patient care. Participate in multi-disciplinary team meetings and case discussions to improve patient outcomes and enhance service delivery. Interpret and evaluate diagnostic test results, liaising with the patient/carer and their GP initiating treatment accordingly. Maintain accurate and up-to-date clinical documentation in accordance with practice policies and legal requirements. Stay current with clinical practices, guidelines, and regulations relevant to advanced practice nursing. Engage in quality improvement initiatives, contributing to the development of practice protocols and standards. Engage in clinical supervision and self evaluation and use this to improve care and practice Provide support to other members of the Nursing Team Demonstrate respect for the dignity of all patients regardless of age, gender, religion, socio-economic class, sexual orientation or cultural group Provide a high standard of care at all times in accordance with the Code of Professional Conduct (NMC 2004) Ensure patient confidentiality at all times in accordance with the Clinical Governance Framework and the Code of Professional Conduct (NMC 2004) Adhere to the requirements of the General Data Protection Regulations (GDPR) This list is not exhaustive Communication: Utilise highly developed communication, negotiation and interpersonal skills Communicate the patients health status using appropriate terminology, format and technology The course of this communication process involves the discussion of complicated issues and the sharing of delicate information on a regular basis Provide health promotion and disease prevention services to patients who are healthy or have acute and /or chronic conditions by giving anticipatory guidance and counselling to promote health, reduce risk factors and prevent disease and disability Uses networks for the purpose of sharing relevant information in a safe and appropriate manner. HEALTH AND SAFETY/RISK MANAGEMENT The NP must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident reporting System. The NP will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990). Assist and work with the Lead for practice Infection Prevention and Control to ensure practice is compliant in its delivery of services, audits and regular checks undertaken to evidence this. Education undertaken and cascaded where necessary to colleagues and external bodies. Liaise with both internal and external colleagues in the monitoring and maintaining of practice Infection control policies and procedures to minimise risk to staff, patients and visitors to the practice. COMMUNICATION & WORKING RELATIONSHIPS The NP will establish and maintain effective communication pathways with the following: Internal: GPs Practice Manager and deputy Nurse Practitioner & Nursing team Pharmacy technicians and local pharmacy Reception and Secretarial team External: Non-medical prescribers forum Community specialist teams Care Quality team HIOWICB H&W PCN team SPECIAL WORKING CONDITIONS The NP is required to travel to care homes and do some home visits The NP will have contact with body fluids ie, wound exudates; urine etc while in clinical practice. REHABILITATION OF OFFENDERS ACT 1994 Because of the nature of the work, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995.
