In this role you will play a vital part in delivering high-quality, patient-centered care to our diverse community. Working within a multidisciplinary team your experience in diagnosing, managing, and treating a wide range of acute and chronic conditions will be essential in ensuring positive health outcomes for our patients within the North Kent community.You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc. This role covers both Dartford and Gravesend, you may need to work in both Livingstone Community Hospital and Gravesham Community Hospital.
