Summary
Under limited supervision, as part of the specialist palliative care services. Is responsible for managing own workload (supporting junior staff as required), defined caseload management and the prioritisation and delivery of care. Provides basic advice on symptom control and psychological support that may require frequent skilled judgements and a level of concentration, with support of the Hospice Registered Nurse. Report untoward incidents, after recording actions taken to prevent reoccurrence, ensuring the completion of relevant documentation in accordance with the Rotherham Hospice Policies. Be able to perform clinical and environmental risk assessments to optimise the health and safety of patients and staff and shares this information with the wider healthcare team. Act as advocate for patients/family referring directly to other MDT members, both within the hospice, community, and hospital, as appropriate. Act as a mentor/role model for junior staff, as a resource, providing clinical support. Share in responsibility for safe custody and administration of medication in accordance with hospice policy and the law, reporting errors without delay. Supporting the work of the Hospice IPU Volunteers. Responsibility for the supervision of Healthcare Support Workers & Volunteers within the IPU team. Responsibility as mentor for pre-registration nursing students, whilst they are on placement. Challenge prejudice and inequalities in access to Hospice provision for individuals. With support from Staff/Senior Nurse, undertake assessments and reviews of the physical, psycho-social, and spiritual care needs of patients with complex life- limiting illnesses, and their family. Initiate medicine management reviews, with support from Senior Nurse/Medic for more complex issues. Provide basic information and advice on prescribed medication regimes, side effects and interactions. Refer patients to other appropriate services, both at Rotherham Hospice and to external teams. Ensure patients needing palliative care receive a high standard of palliative care, aligned to the principles of Gold Standards Framework, Ambitions for Care, and an End-of-life Care Pathway.
