Summary

Job responsibilities Main responsibilities of the Primary Care Network Nurse include the following areas Community, Neighbourhood Care & Health Inequalities Maintain partnerships across primary care, community services, local authority, and voluntary sectors Engage with underserved communities to co-produce solutions and support equity of access Assist to deliver and evaluate programmes to reduce health inequalities Assist with proactive clinics in the community using a population health approach Use risk stratification and data to target interventions effectively Clinical Duties and Responsibilities within the Primary Care Network Manage and monitor chronic diseases by regularly assessing patients, reviewing treatments, and coordinating care within the Primary Care Network. Perform health screenings to identify risks and support preventive care within the Primary Care Network. Administering injections and providing guidance in accordance with guidelines Work independently or alongside the Primary Care Network team Care Home and Frailty Support Provide comprehensive health assessments and care planning for frail and housebound patients Participate in multidisciplinary ward rounds in care homes Offer proactive support for palliative care, dementia, and complex long-term conditions Support patients and carers in managing conditions and navigating services Deliver opportunistic health promotion and wellbeing advice Contribute to the development of personalised care and support plans Maintain effective communication with wider clinical teams and families Infection Control, Governance & Policy Management Support PCN nursing team with Infection Prevention and Control Assist with IPC audits, training and policy compliance across the Primary Care Network Participate in clinical audits and quality improvement initiatives Support shared learning following significant events or near misses Support with national or local policy changes affecting clinical delivery Ensure compliance with data security and Information Governance Other To maintain accurate clinical records in conjunction with extant legislation. There may, on occasion, be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: HEALTH & SAFETY The post holder must be aware of the responsibilities placed on them under the health, safety and security as defined in the Primary Care Network Health & Safety Policy to include: Using personal security systems within the workplace according to Primary Care Network guidelines Identifying risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazard Reporting potential risks identified. The post holder will be expected to be flexible in relation to duties undertaken and location of work where required by the Primary Care Network. Must have access to own transport and be able to travel across sites and into the community. Flexibility to work outside of core hours where required.
