Summary

We are seeking a skilled and experienced Services Practitioner to join our team. In this role, you will lead on provision of Floating Support Cover delivering information, advice, guidance, care and support across all Humbercare services as and when required on a planned rota. This planned rota will be spread across 40 hours a week, and will involve working nights and weekends.

Key Responsibilities

To provide information, advice and guidance and person-centred care and support to customers across all Humbercare services as required and in accordance with individual service contract requirements;
To engage with customers to support individuals to secure, maintain and achieve independent living whilst ensuring health and wellbeing needs are being met;
The co-provision of support plans, risk management plans, crisis intervention and engaging with customers in order to deliver positive outcomes in services that offer involvement and empowerment for customers;
To provide support in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Service Managers;
To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.

Essential Requirements

The successful candidate will have:

QCF Diploma Level 3 in Health & Social Care or be working towards this;
Level 2 Housing Management qualification (or be working towards this).

Due to the nature of the role and its requirements, a full driving licence and access to your own transport will be necessary.

Skills and Experience Required:

An understanding of the social housing environment, including emerging government policy;
Experience of working in a social care or similar environment;
Knowledge of the issues faced by service users that have been homeless, rough sleepers or gone through the criminal justice system;
Knowledge of the Care Quality Commission, and the Key Lines of Enquiry and fundamental standards;
Knowledge of varying complex needs and their impact on the ability for a person to access housing or maintain independent living;
Working experience of delivering care and support to people with their mental health and wellbeing;
An understanding of the Mental Capacity Act and other legislation relevant to the role;
Knowledge of welfare benefits and housing benefit entitlement, local statutory and voluntary support agencies, and experience of dealing with complex service users;
Excellent organisational skills, and the ability to effectively manage a caseload;
Excellent communication and IT skills, inclusive of Microsoft Office;
Ability to deal with difficult situations with tact and diplomacy.
