Summary
We are recruiting a passionate and proactive Clinical and Operational Lead for a new Neighbourhood Proactive Care Service.
This is an exciting opportunity to play a key role in a new teamdelivering community-based, person-centred care for residents.Thefocuswill beon holistic assessments, care coordination, addressing health inequalitiesand delivering on the strategic ambition of a‘neighbourhood health service’.This role will have both an operational management and patient facing element.
This service will see the shiftfrom reactive caretoproactive identificationbyusing risk stratification searcheswithin Primary Care records.The searches willidentifya cohort of patients who wouldbenefitfromtargeted interventiontoimproveoutcomes, their overallexperience and reduce the likelihood of further unplanned admissions. Theservice willoperatealongsidea number ofother acute and community services,with the ability to step patients upin toappropriateservices -including thehighly successfulvirtual ward models in South and West Hertfordshire.
This rolepresents as an excellent opportunity to work in a topical area of service delivery within the NHS right nowand willprovidethe opportunity to gain service developmentskills.
You'llbe responsible for operational delivery of the service, leading and managing a multidisciplinaryteam.
You will be working closely with a programme manager, testing new models of care, identifying interventions which make the most difference to this patient cohort, and evaluating the success of the service. Clinically, you will be from a nursing, physiotherapy, or occupational therapy background and will have appropriate clinical experience to be able to supporta cohort ofresidentswith moderate to severe frailtywho have had 2 or more unplanned admissions within the last year.
