Description
We are seeking a dedicated and passionate Social Worker to join our Adult Learning Disability Team. Responsibilities
Assess the needs of adults with learning disabilities and develop tailored support plans.
Provide direct social work intervention and advocacy for clients and their families.
Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
Monitor and evaluate the effectiveness of support plans and make adjustments as necessary.
Facilitate access to resources, services, and community involvement opportunities.
Conduct regular reviews and assessments to maintain compliance with legal and regulatory requirements.
Educate clients and families regarding rights, available services, and coping strategies.
Requirements
Hold a recognized social work qualification and be registered with the appropriate regulatory body.
Demonstrated experience working with adults with learning disabilities or related fields.
Strong understanding of relevant legislation, including safeguarding and mental capacity.
Excellent communication and interpersonal skills for effective client engagement.
Ability to work collaboratively within a multidisciplinary team environment.
Proficient in case management and documentation practices.
Commitment to ongoing professional development and best practices in social work.

End Time: 17:00
Pay Per Hour: £24.80
