Summary

Job summary: Receive, assist and care navigate patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone. Job responsibilities: Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice. Deal with all general enquiries, explain procedures and make new and follow-up appointments. Using your own judgement and communication skills, ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner. Care navigation signpost patients to the most appropriate services. Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, and ensure procedures are completed. Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery. Action medication queries, repeat prescriptions, online repeat prescription requests in a prompt manner. Liaise with patients and pharmacies to resolve issues with medications. Enter patient information on to the computer as required ensuring clear and concise detail. Ensure that all new patients are registered onto the computer system promptly and accurately, ensuring patients are invited for a new patient medical where appropriate. Enter patient-related documents onto their medical record using read codes as agreed by the clinical team. These may be received via post or electronically via email or SystmOne. Action emails, tasks and other forms of communication received in to the practice. Send referral letters and other communications to specialists and external healthcare providers. Coordinating referrals to specialists and other healthcare services as directed by the GPs. Provide cover for colleagues in the event of sickness and shortage which may involve changing times/days of shifts. Assisting in the preparation and distribution of medical reports and summaries
