Summary
Provide effective case management to caseload in the community. Develop a therapeutic working relationship with service users whose difficulties have historically been described by the term personality disorder, and facilitate focussed treatment interventions as appropriate. Collaborate with service users, carers and other service providers to develop effective care plans taking responsibility for implementation and regular reviews. Ensure effective care co-ordination and communication within care teams, the wider Network and others involved. Ensure that crisis plans are in place, up to date and circulated. Undertake risk assessment and risk management for individual service users and to provide advice to other professions on risk assessment and risk management. Offer psychologically informed assessment to young people referred for care coordination. Undertake regular office duty Attend monthly clinical supervision Attend monthly management supervision Attend weekly clinical meeting Attend monthly Team Development sessions Contribute to the collection of outcomes measures and take part in service evaluationThe service actively maintains a reflective, supportive and learning culture to support the work it does. Opportunities are provided to develop experience and skills and staff are supported to develop within their professional roles. Team members are provided with training and access to both regular clinical and managerial supervision. You will be required to travel by car within Leeds on a regular basis.
