About Us We are a dynamic and welcoming GP surgery serving over 8,300 patients. Our practice offers a wide range of primary care services, including chronic disease management, preventive care, health promotion, family planning, and same-day urgent appointments. As part of a local Primary Care Network (PCN), we collaborate with healthcare providers across South Hams to deliver integrated, seamless care. Our experienced team includes 5 GP Partners, 4 Salaried GPs, 1 Advanced Nurse Practitioner (ANP), 3 Practice Nurses, 1 Nursing Associate, 3 Healthcare Assistants (HCAs), and 2 Phlebotomists, supported by a dedicated reception and administrative team. Job Purpose We are seeking an experienced and enthusiastic Practice Nurse to join our team. The successful candidate will work collaboratively with GPs, the ANP, nursing colleagues, and support staff to deliver exceptional patient care and improve health outcomes. Key Responsibilities Manage chronic disease clinics such as diabetes, asthma, COPD, hypertension, and cardiovascular disease. Administer immunisations following practice and national guidelines. Undertake cervical smears, wound care, and other treatment room procedures. Provide family planning advice and support. Support health promotion initiatives, including smoking cessation and weight management programs. Conduct routine health checks and patient reviews. Maintain accurate patient records and adhere to practice protocols and standards. Person Specification Registered Nurse with the Nursing and Midwifery Council (NMC), preferably with primary care experience. Proficient in chronic disease management and treatment room procedures. Strong commitment to patient-centred care and clinical excellence. Ability to work independently and as part of a multidisciplinary team.
