Summary
JOB SUMMARY This job description is an outline of the main duties of the post. The post holder will be required to undertake other duties commensurate with the grade as directed. The content of this post will be reviewed in consultation with the post holder when necessary and in line with service developments. KEY RESULT AREAS: Job Purpose To take clinical leadership responsibility for a specialist Breathe Service for the treatment of patients with a variety of acute and long-standing respiratory problems including those with highly complex presentations. To take overall managerial leadership and be responsible for the delivery of the Breathe in the Community Service in line with the service specification. To undertake and oversee specialist assessments, treatment, and advice to patients and carers ensuring that the service formulates a treatment plan for all patients referred to the Breathe Service, this includes within a crisis situation, mobilising and link with relevant service pathways in the best interest of the patient (e.g. same day emergency care, same day diagnostics, point of care testing). To act as a source of expertise working at an advanced level of practice on the management of respiratory conditions providing an advisory service to health care professionals including AHPs, GPs, secondary care professionals and consultants. To provide clinical leadership and overall day to day managerial responsibility for the effective delivery of the Breathe in the Community Service including the line management of all clinical and support staff employed within the service. Job Dimensions To manage other healthcare professionals whilst having responsibility for the effective delivery of the Breathe Service for patients with respiratory conditions who are supported in the community.
