Summary


The West Home First Hub team are pleased to advertise a band 6 Clinical Co-ordinator opportunity within our friendly and supportive team based at St James Clinic, Kings Lynn.

The aim of our service is to provide evidence based triage and case management of Discharge to Assess and Admission Avoidance referrals. You will be working closely within an integrated Health and Social Care team to support patients with complex health and social needs.

We are looking for a dynamic, forward-thinking Health or Social care professional with effective communication, leadership, and organisational skills. The skill set for this role may not be part of your current portfolio, however we can offer support and leadership, and are keen to discuss the role with interested candidates.

To contribute to the review and assessment of health and social care needs for the Discharge to Assess pathways providing safe discharge plans and support flow from the acute hospital.

To triage, assess and co-ordinate responses to Admission Avoidance referrals.

To undertake Trusted Assessments for patients within the acute Trust requiring pathway 2 discharges

· Triage and allocation of referrals for patients on Discharge to Assess pathway, assessing the patient’s needs, determining the priority of care, processing and allocating referrals accordingly, liaising with multidisciplinary team to ensure timely responses to referrals on an individual patient needs-based basis, utilising the knowledge, skills and expertise of others in the integrated team.

· To assess and prescribe care packages for patients with long term conditions, the frail and elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.

· To work within the integrated team to facilitate early discharge from hospital.

· To work within the integrated team to prevent unnecessary admission to hospital.

To work, liaise and communicate with all health care professionals, and statutory / non statutory agencies to provide a seamless, integrated service to our service users on the Discharge to Assess pathway and in community teams
