You will need to register if you do not already have an account To use own initiative to support children and families in their homes and other environments by carrying out developmental assessments and planned individual programmes of care under the leadership of SCPHN To maintain accurate records as per Trust policies and procedures utilising IT resources To effectively communicate with clients, health care professionals and the wider team including local Derbyshire County Council Plan individual programmes of care and interventions in conjunction with the SCPHN and in partnership with the client. Provide a face to face service to families that is personalised in response across a variety of community settings and the patients home. Undertake health promotion activities under the supervision of the SCPHN Provision of enhanced support to children and families Support children with school readiness activities Promote health enhancing activities in partnership with other health professionals, statutory, voluntary, community organisations and members of the local population.
