Summary
With the knowledge and approval of the Reablement Practitioner, to work with service users, in their own homes, as detailed in their service user plan and agreed outcomes by providing active support to enable them to live as independently as possible and take part in the activities of everyday life To be an active part of the assessment &reablementprocess in delivering and reviewing the service user plan To work as part of the multi-disciplinary team liaising with other professionals and services as necessary to provide the best possible care to service users and their carers In view of the nature of the work, no information concerning service users must be communicated to anyone who is not either employed by the Department or attending the service user in a professional capacity
