Rydym ni’n chwilio am ddau unigolyn hynod frwdfrydig i ymuno â'n tîm ac i arwain a datblygu ein tîm o Swyddogion Asesu Datrysiadau Tai i atal a dileu digartrefedd yng Nghonwy.
Mae hwn yn gyfnod cyffrous i ymuno â'r gwasanaeth Datrysiadau Tai yng Nghonwy. Rydym ni wedi ailstrwythuro’r gwasanaeth yn ddiweddar ac yn bwrw ymlaen â'n Cynllun Pontio Ailgartrefu Cyflym, gyda'r nod o wneud digartrefedd yn rhywbeth prin, byr, nad yw’n digwydd eto. Mae hyn yn cynnwys canolbwyntio mwy ar ymyrraeth gynnar ac atal digartrefedd drwy sefydlu Gwasanaeth Atal newydd o fewn y gwasanaeth Datrysiadau Tai.
Mae gennym ni ddwy swydd Arweinydd Tîm lawn amser ar gael, gydag un swydd barhaol ac un swydd am gyfnod penodol o ddwy flynedd.
Bydd yr Arweinydd Tîm parhaol yn arwain tîm o Swyddogion Asesu Datrysiadau Tai sy’n gweithio gyda phobl ddigartref a phobl sy’n byw mewn llety dros dro, ac mae’r rhain yn cynnwys swyddogion arbenigol sy’n canolbwyntio ar bobl ifanc a’r rheiny sy’n cael eu rhyddhau o’r carchar. Byddwch yn edrych ar ddatblygu cynlluniau symud ymlaen ac yn cefnogi’r tîm i wneud penderfyniadau statudol a rhedeg Gwasanaeth ar Ddyletswydd ar gyfer pobl sy’n dod yn ddigartref.
Bydd yr Arweinydd Tîm cyfnod penodol yn gweithio ochr yn ochr â’r Rheolwr Datrysiadau Tai i ddatblygu, creu a recriwtio Tîm Atal Digartrefedd newydd a fydd yn gweithio gydag aelwydydd sydd mewn perygl o fod yn ddigartref. Byddwch yn arwain eich tîm i ddatblygu cysylltiadau a rhaglenni atal i sicrhau gwasanaeth atal llwyddiannus a lleihau digartrefedd yn y sir. Mae’n bur debyg y bydd y cyllid yn parhau os yw’r cynllun peilot dwy flynedd yn llwyddiannus.

Do you want to make a difference to individuals in housing need in the County?
We are looking for two extremely motivated individuals to join, lead and develop our team of Housing Solutions Assessment Officers to prevent and end homelessness in Conwy.
This is an exciting time to be joining the Housing Solutions service in Conwy. We have recently restructured as we move forward with our Rapid Rehousing Transition Plan, working to make homelessness rare, brief and unrepeated in the County. This includes an increasing focus on early intervention and prevention of homelessness with the establishment of a new Prevention Service within the Housing Solutions service.
We have two Team Leaders opportunities available, both full time, with one permanent and one fixed term for two years.
The permanent Team Leader role will be leading a team of Housing Solutions Assessment Officers who are working with those who are homeless and in temporary accommodation, this includes specialist officers focusing on Young People and those leaving prison. This is a fast paced role looking to develop move on plans and support the team with statutory decision making and running a Duty Service for those homeless on the day.
The fixed term Team Leader role will work alongside the Housing Solutions Manager to develop, build and recruit a new Homeless Prevention Team that will be working with households who are at risk of being homeless. You will lead your team to develop prevention tools and contacts with the aim of near universal prevention and thus reducing homelessness in the county. Funding will likely continue should this two year pilot be a success.
