Summary
Hold face to face and telephone clinics for patients requiring medication reviews. Provide telephone support for patient queries relating to medication. Undertake structured medication reviews with multi morbidity and polypharmacy and implement changes whilst ordering relevant monitoring. Collaborate with the wider healthcare team to improve safety of medicines ordering and administration. Provide support for the pharmacy technician for management of medicines following hospital discharge and clinic visits.
