Summary

Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated.

Hours: 37.5 hours per week
Salary: £25,027.20 per annum

Based at: Gwynedd

Role Purpose:

• Provide support and guidance to all participants that have been registered onto the project.
• To develop ongoing action plans with participants which will be reviewed on a regular basis.
• To provide one to one support and group activities to aid the development of a participant in their recovery.
• Work alongside the team structure in delivering services and outcomes for the scheme.
• To achieve project targets and outcomes set by Welsh Government.
• To support the team leader and referral agents in raising awareness of the service.
• To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance misuse.
• Provide support and supervision to volunteers.

• Darparu cefnogaeth un i un a gweithgareddau grŵp i gynorthwyo datblygiad cyfranogwr yn ei adferiad.
• Gweithio ochr yn ochr â'r strwythur tîm i ddarparu gwasanaethau a chanlyniadau ar gyfer y cynllun.
• Cyflawni targedau a chanlyniadau prosiect a osodwyd gan Lywodraeth Cymru.
• Cefnogi'r arweinydd tîm a'r asiantau cyfeirio i godi ymwybyddiaeth o'r gwasanaeth.
• Gweithio mewn partneriaeth ag ymarferwyr eraill i ddarparu ymyriadau a chymorth effeithiol i bobl ag Iechyd Meddwl a/neu gamddefnyddio sylweddau.
• Darparu cefnogaeth a goruchwyliaeth i wirfoddolwyr.
