We are looking for an enthusiastic Band 6 Transition Nurse to work within the Children’s Community Nursing Team (CCNT) in Oldham. This is a full time 12 months fixed term / secondment post; Monday to Friday, with some weekend, bank holiday and evening work as required to meet the needs of the service. Successful candidates must have demonstrable experience at Band 5 or Band 6.
Community experience is desirable however consideration will be given to applicants without should suitable evidence of transition or complex care be provided.
The CCNT provides care closer to home for babies, children and young people across the Oldham area who have a diagnosis of a short term/acute illness; chronic/long term conditions; complex and additional health needs and end of life care. The postholder will work as part of the CCNT, working closely with key stakeholder partners from community, acute secondary and tertiary settings, local authority and the voluntary sector.
The (CCNT) provide a weekend and out-of-hours service (including 24/7 end of life care) and the post holder will be required to be flexible and adaptable to the needs of the service.
The post holder will be located at the Integrated Care Centre, Oldham.
