Summary

To work in partnership with service users in restoring their self-confidence and independence under the direction of the case co-ordinator.To demonstrate the safe use of and provide equipment, adaptations and alternative methods or coping strategies to increase or maintain peoples independence and/or assist carers (under guidance of therapist)To measure client goals in line with the support plan and to adjust support plans as agreed with case co-ordinator.To work closely and undertake joint visits to service users homes with colleagues from multi-disciplinary teams as directed.To provide cover for the business administration function in the team and undertake support duties as directed.To identify situations where clients are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults.To facilitate client choice and empowerment through promoting independence, in particular through the timely provision of information, advice and support to ensure clients maintain independence for as long as possible.To ensure the highest level of date quality for all information recorded, ensuring that the information is consistently timely, accurate and complete to ensure the service meets all trust policies. CLINICAL RESPONSIBILITIES Carry out client risk assessment as agreed with the occupational therapists and physiotherapists and provide feedback on clients physical and occupational performance and skills deficits.Carry out client treatment and ongoing rehabilitation as agreed with the occupational therapists and physiotherapists and as per service protocol and feedback on clients physical and occupational performance.Provide client instruction, direction and guidance during assessment and intervention and adjust own clinical interventions based on clients needs during the intervention. To evaluate effectiveness of the intervention in discussion with supervising therapist, and modify programme accordingly.Provide above client interventions in the patients home or in a community setting as appropriate.Use communication, reasoning and negotiation skills to establish a therapeutic relationship with clients and identify barriers and possible solutions to communication difficulties.Feedback effectively to multi-disciplinary team and external agencies on clients performance and progress towards identified rehabilitation goals.Comply with all relevant national and local policies and procedures.Respect the individuality, values, cultural and religious diversity of clients and contribute to provision of a service sensitive to these needs. Exercise good personal time management, punctuality and consistent reliable attendance. Undertake responsibility for management of peripheral store and weekly ordering procedures and organise cover for this when required. TRAINING/EDUCATION RESPONSIBILITIESReview and reflect on practice and identify areas for self-development through participation in the annual appraisal system and regular clinical and professional supervision.Attend courses to enable achievement of appraisal objectives and to share this knowledge with peers on return.Attend and actively participate in RRRT monthly meeting and other relevant meetings.
