Job Purpose
To provide compassionate, person-centred care that promotes dignity, independence, and respect. Accountabilities
Delivering The Highest Standards of Care to all Service Users
- Provide personal care and support as detailed in individualised care plans.
- Promote independence while preserving dignity, privacy, and choice at all times.
- Ensure a safe environment by adhering to risk assessments and support plans.
- Support with meals, household tasks, medication, mobility, and social interaction.
- Work respectfully in each person’s home and adapt to individual needs and preferences.
- Develop trusting, respectful relationships with service users and their families.
- Communicate openly and effectively with all parties involved in care delivery
Supporting Organisational Excellence
-Work flexibly to meet changing care needs and visit schedules.
- Maintain accurate, timely care records in line with legal and policy requirements.
- Participate in reviews, case conferences, and supervision as required.
- Report concerns immediately including safeguarding, cancellations, or changes in wellbeing.
- Attend training, team meetings, and development sessions to support continuous improvement. Working with Families, Peers, and Professionals
- Attend multidisciplinary meetings and engage professionally with all stakeholders.
- Report updates or risks promptly to the office team.
- Handle service user property and information responsibly and ethically.
- Undertake other duties as requested that align with the needs of the service and role.

Person Specification
Essential Requirements
- A genuine passion for care and supporting others with dignity and respect.
- Ability to demonstrate empathy, emotional intelligence, and sound judgement.
- Effective communication skills – verbal, written, and active listening.
- Ability to recognise and report safeguarding and poor practice concerns.
- Knowledge of empowerment, inclusion, and the rights of individuals.
- Ability to work independently and as part of a supportive team.
- Strong organisational skills and reliability in attendance and punctuality.
- Full UK driving licence and access to a roadworthy, insured vehicle for work use.
- Willingness to undergo an Enhanced DBS Check.
Desirable Attributes
- QCF/NVQ Level 2 or above in Health & Social Care.
- Previous experience supporting people with dementia, learning disabilities, autism, etc.
- Completion of specialist training in relevant health or behavioural conditions.
