Communication and Record-Keeping Use clear, sensitive communication to obtain informed consent and ensure patient understanding, adapting style to different health literacy levels and cultural backgrounds. Maintain accurate and contemporaneous patient records in SystmOne, upholding GDPR and confidentiality requirements. Attend practice and educational meetings, and contribute to clinical audits, guideline development, and team case reviews. Act as a patient advocate when liaising with secondary care, community teams, and social services. Leadership and Team Development Mentor and support HCAs, Nurse Associates, and student nurses. Information and Technology Use SystmOne proficiently for patient coding, record-keeping, QOF reporting, and data retrieval. Ensure strict compliance with data protection, patient confidentiality, and Freedom of Information requirements.
