Summary
For further information on this role, please see the attached detailed Job Description and Person Specification :- To sustain a high-quality specialist service for the patient, from diagnosis through all stages of the disease and actively provide optimum liaison and continuity of care. To discuss all treatment options, including research-based treatments for colorectal cancers with sensitivity, knowledge and expertise to enable patients to make an informed choice. To act as patient advocate when appropriate and respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements. In collaboration with Clinical Nurse Specialist colleagues, to formulate and/or revise relevant policies and to regularly review this following change/implementation. To work within the procedures and policies of the Trust. In conjunction with Clinical Nurse Specialist colleagues, to develop assessment tools that will ensure an appropriate level of nursing intervention so that patients with complex needs are referred to the appropriate specialist/s. To initiate maintain nurse-led and telephone clinics whereby patients are provided with information and support and the problems and needs of patients and their carers are assessed and appropriately channelled and referred to other services as required. To initiate, run and evaluate appropriate group support in conjunction with other relevant professional disciplines.
