Summary
To deal with patient prescription queries, discharge letters, upgrade patient records accordingly, and update medicines. To call the hospital to confirm changes and discuss with the clinical pharmacist if needed. To set up the patient's repeat prescription and synchronise the medication. Deal with out-of-stock medicines and find suitable alternatives. To switch branded medicines to generic medicines to improve cost, following BNF, NICE, GMMMG, and local guidelines before making any changes. To help the practice with QOF work and primary care incentive schemes. IIF (Investment and impact fund) work in practice. Medicine monitoring and arranging blood tests for patients. Sending alert messages to patients to update their BP, BMI, and other information required by practice .Other tasks appropriate to the role.
