Summary
We have an exciting opportunity for an enthusiastic, Specialist Diabetes Dietitian to join our team delivering innovative dietetic services to people with diabetes at Wembley Centre for Health and Care and Willesden Central for Health and Care
This is a broad summary of the role detailing key areas of operation and responsibilities – further detail will be provided in the Main Duties and Responsibilities section, below, which provides more detail with the Job Description.
You will also be supported by an experienced team from the Community Diabetes Specialist Service Team and the Nutrition and Diabetes Team lead who hold regular peer support meetings and provide 1:1 supervision to facilitate clinical excellence and personal development.
• To be the Dietetic lead within the Community Diabetes Service to provide a specialist clinical Dietetic Service that contributes to the delivery of the diabetes specification within the well-established multidisciplinary Community Diabetes Service in line with the strategic direction of the Service and Trust objectives.
• To work within a multi-professional framework and contribute to a first-class service through evidence-based clinical practice, management, education, audit, and research.
• To provide specialist assessment and treatment of clients with Type 2 diabetes, liaise with other agencies and health professionals, and maintain accurate clinical records.
• To set and work towards individual and team objectives that reflect service plans and work within the Clinical Governance Framework.
• To utilise project management skills and work in partnership with service users, community agencies and professionals to plan, develop, implement, and evaluate services provided.
• To lead on specific dietetic clinical standards, audit, and/or research projects within the multidisciplinary Community Diabetes Service to inform Clinical Governance strategy and service development.
