We are seeking an enthusiastic individual to take up a band 5 Occupational Therapist (OT) role within dementia and frailty service at Sage Ward Juniper Centre. This is a 18 month maternity cover.
You will be based on Sage ward which is a 17 bedded male dementia ward, working as a key member of that team where you will manage, develop and deliver occupational therapy (OT) provision to a designated caseload. You will focus on occupational performance, complex mental health presentations, risk management and evaluating outcomes aligned to personalised care plans. You will implement and facilitate individual and group programmes of clinical intervention in collaboration with Occupational Therapy assistants, activity workers and other professionals for effective recovery and discharge.
Whilst you will be based with a specific ward team, as a member of the wider occupational therapy service there may be occasions when flexibility of location is required to accommodate clinical need. The post holder is expected to work Monday - Friday 8am-4pm, working closely with the band 7 Occupational Therapist, Ward Manager, Matron, Lead Occupational Therapist and the ward multidisciplinary team.
The Occupational Therapist will be required to undertake comprehensive OT assessments using a number of standardised tools and models, including MOHO (model of human Occupation). The OT will apply formulation skills in setting appropriate goals and work together with the service user in achieving these goals by application of appropriate interventions.
A core role of his post is inputting into the division wide OPIP service which offers interventions in group and one to one settings for service users with organic and/or mental health needs.
The OT will be directly supervised by the band 6.
You will have regular managerial, professional and clinical supervision. There are also training opportunities within the Trust and support of continuing professional development.
